Release Form, Registration Form & Gear List
Please select/click, print, fill out & sign forms below. Release forms are required for all participants. If necessary, you can bring forms with you on the day of your trip and hand them to your guide before your trip starts. No faxed or emailed forms accepted.

Your Confirmation Email will provide you a detailed location where to meet. You will have two options. The confirmation email will indicate which option below:
- North Point Parking lot – Take NE Crooked River Drive until it ends and find parking. Meet your guide(s) at the Smith Rock State Park information board and fee station.
- Smith Rock State Park Welcome Center – Take NE Crooked River Drive and find parking when you see the Smith Rock State Park Welcome Center (Green Yurt and Bathrooms). Meet your guide(s) at the wooden fence in front of the bathrooms by the road.
